Some basic paperwork needs to be filled out before beginning our initial meeting. Please download, print, and complete the appropriate forms linked below and bring them in with you to your first appointment.  

• Initial paperwork for new therapy clients

• Release of Information - only required if you need to authorize me to contact or be contacted by a third party

• Discounted Fee Agreement - if you cannot plausibly afford the standard psychotherapy fee, please talk to me about my current availability before completing this form (for ongoing psychotherapy only, not applicable to assessment/testing)
